Health Coaching at Evolve

Here at Evolve Direct, we believe in helping people reach their goals. That is why we have a full-time, certified Health and Wellness Coach, Leah Canale.

According to Dr. Michael Freedman, founder of Evolve, adding health coaching is critical for Primary Care practices looking to truly help people make important lasting changes.

“Just prescribing medications and telling people ‘lose weight’ is not working,” according to Dr. Freedman.  “We can, and should, do more to help people get healthier and stay well.” 

Pricing

Health coaching is FREE to our members for 30 days. Whenever you are ready to start, we will work closely with you for 30 days.

After the first 30 days, if you want to continue getting expert health coaching, members can sign up for an additional 90-day program for only $150/month for 3 months.

We also offer an alternative plan where each follow-up visit is just $40 per visit (30 minutes) or $75 per visit (60 minutes).

The bottom line? You choose. More intensive health coaching or a la carte. Or none. Our philosophy at Evolve is to make available any tools you might need to become healthier and happier but at the same time, respecting that each of us has a right to chose where, when and how we’ll pursue those goals.

How Does a Health Coach Help?

Health coaches help with a lot of the problems that frustrate people. They are primarily used to help with weight loss, improving energy, and forming better eating habits for individuals and families. 

Health coaches use personalized programs to help people break out of a rut. They do this by educating, mentoring and motivating people to make more positive health decisions.

Setting Goals

According to Leah Canale, patients from Evolve start by describing their current lifestyle and
what changes they may want to make. Next, she offers some education on ideal and reasonable goals and finally, together, they make a set of goals with a focus of “doable steps”.

Leah emphasizes, “Goals really need to be reachable. We might have a ‘final goal’, but our focus will be on what we can accomplish tomorrow, this week, this month. One step at a time.”

Once someone has a clear set of goals and the steps they need to take to get there, health coaches act to motivate, remind, encourage and cultivate ongoing positive health choices.

As everyone knows, it is way too easy to get sidetracked or “slip” off the path of getting healthy. All too often, people will get frustrated with themselves and feel “I can’t do it” and give up entirely. Leah feels that the ongoing motivation, and just knowing someone is there, with them and rooting for them, can be enormously helpful when you are trying to get back to making the healthy choices again.

The other important role that health coaches provide is education and filtering. We are constantly bombarded by information from magazines, the news and the internet with reports of one thing being healthy and then other reports of the same thing is deadly. A health coach can be the person you reach out to when unsure which direction to go, and who to believe.
