Evolve Direct Primary Care is an innovative Primary Care | Urgent Care that is affordable, accessible and uses technology to assist patients in achieving high quality, convenient medical care.
Evolve Direct Primary Care’ core guiding principles:
• A hospitality culture with emphasis on service. Members have on-time visits, same day appointments, and reliable follow-up. The clinic is modern with a relaxing cafe-like reception area.
• Technology-driven for effective and efficient communication. Patients can be assisted with virtual visits and can use online and mobile tools including text for appointment scheduling, prescription renewals, and accessing personal health records including health provider notes.
• Team-oriented, evidence-based, cost effective care. The membership model removes incentives of multiple visits and rewards the delivery of quality healthcare in a timely and convenient manner by a professional medical staff.
• An affordable membership model. Members pay $35 per month and $25 per visit. Children ages 12-25 years old qualify for a free membership with their parent’s membership. Non-members pay $105 per visit.
Although we don’t bill insurance for you, you may be reimbursed by your insurer for visits. Any test, specialist or service outside of our primary/urgent care office is still done through your insurance plan. If you have a deductible, it’s actually possible to save money by joining our practice.
Our individual providers have been hand-selected from the best of the best. Each provider is considered by many in Annapolis to be one of the best primary care or urgent care providers. They all have impeccable credentials. Our Medical Assistants are likewise as good as it gets. Each with nearly 20 years experience, they are adept at all matters necessary for primary care or urgent care including phlebotomy. Lastly, but not least, is Jazmin, our office manager. A remarkable woman with a natural gift for making people feel more comfortable and at the same time, knowing what it takes to get the job done!
